Consent Agreement to Access Private Property

By signing below, I hereby certify that I am the owner or lessee/renter of the above-described private property and give consent and agree to the following:

• I am the owner or lessee/renter of the above-described property. If the owner or lessee is a business entity, I certify that I am authorized officer, manager, or representative that has authority to give consent on behalf of the business entity.

• Any employees and authorized representatives of the Idaho Department of Environmental Quality (DEQ) may enter and have continued access to the above-described private property to conduct inspections or investigations in accordance with Idaho Code § 39-108.

• DEQ may collect reasonable samples or other materials that are necessary to conduct the inspection or site visit.

• My signature constitutes consent to access the above-described private property for the purposes of the Property Rights Protection Act, Idaho Code § 18-7101, et seq.

• I give this written permission voluntarily, with knowledge of the right to revoke my voluntary consent at any time, and without threats or promises of any kind.
