Annual Report Form and Instructions (pdf 83 kb, 4 pages)
Report to DEQ on amounts of waste generated and managed during the 2008 calendar year, as required by Idaho Code 39-4411(4). Due January 31, 2009. File paper form (link above) or electronically at www.deq.idaho.gov/Applications/hwar/ login.cfm. To obtain report forms for previous years, contact René Anderson at (208) 373-0210.

Biennial Hazardous Waste Report Form and Instructions (EPA Form 8700-13A/B)
Report information on quantities, type, and management of hazardous wastes generated and received from off-site.
Emergency Information Form (Word format)
Use this form to record emergency contacts and locations of emergency equipment.

Local Authorities Letter (Word format)

Send this letter to advise local fire authority of hazardous wastes on-site and help ensure proper response in the event of an emergency.

Manifest Form and Instructions (EPA Form 8700-22)
Report movement of hazardous waste off-site from generator to permitted hazardous waste treatment, storage or disposal facility.

 Non-Municipal Solid Waste Landfill Facilities (NMSWLF)

Preapplication Meeting Information Form: New or Expanding of NMSWMF (Word)
Provide preliminary information before meeting with DEQ and local public health district to discuss a request to construct or expand.

 Siting

Site Approval Application: New or Expanding NMSWMF (Word)
Demonstrate compliance with applicable siting requirements and apply for DEQ approval.

 Design

Design Plan Approval Application: New or Expanding NMSWMF (Word)
Demonstrate compliance with applicable design requirements and apply for DEQ approval.

 Operation

Operating Plan Approval Application: New or Expanding NMSWMF (Word)
Demonstrate compliance with applicable operating requirements and apply for approval by local public health district.

 Closure

Request a variance from siting, design, operating, and/or closure/post-closure care requirements.
 
 Underground Storage Tanks

Notification for Underground Storage Tanks:  pdf format  |  Word format

Notify DEQ of upcoming installations and replacements, new facilities, upcoming tank closures, and closed facilities.  Notification for upcoming installations and closures must be made 30 days in advance.  Notification for replacement piping must be made 24 hours in advance.

Release Reporting Form (pdf)

For underground storage tank system releases, owners or operators must report the release source and cause.
